I have breastfed all 4 of my girls, all for different amounts of time.  As for when to start pumping, I definitely wouldn't worry about it in the hospital, as your milk won't really be in yet, and baby needs the colostrum.  It takes a few days for your milk to come in, but believe me, you will know when it does.  That was a real shock for my friend because she thought it was going to be instantaneous.  There are all kinds of different theories, ideas on when is best to pump.  Often, I would pump on one side while they nursed on the other.  My friend was told to feed him and then pump afterward, but that can be a problem because the baby never gets the hindmilk, which will keep him/her fuller.  If they get too much foremilk, it can create issues with spitting up as well.  I think Judy's idea of pumping and then feeding them is also good.  Basically, it's a bit of trial and error.  I know people who would get up and pump in the middle of the night, but I can say that I have never done that.

In regards to bottles, it really depends on the baby.  My first would not ever take a bottle when I was away from her and would just wait until I got back.  She wasn't crabby about it -- she just didn't want it.  My second and fourth really liked the AirFlow ones (which I think are Playtex).  My recommendation is to get a couple of different sample packs and try different ones out.  I would start offering the bottle somewhere around 2 weeks of age, maybe once or twice per week.  However, it needs to come from your husband or someone else, and it's best if you're not in the room.  Babies smell their mamas, and that can keep them from taking a bottle.  Also, it wouldn't hurt to pump in another room at the same time that they are having a bottle so that you don't become engorged.  For me, the most important thing was to make sure that we got the slowest flow nipple possible for the bottle.  The harder they have to work at getting it out of the bottle, the more likely they are to continue breastfeeding.  My daughter was still using the Level 0 or whatever nipple, even at 1 year old.
